Thank you for this very clear ethic...Dear Vicente,<br /><br />Thank you for this very clear ethical position. I hope this reform will be stopped. It's not only a Spanish problem, since xenophobia is expanding everywhere in Europe. <br /><br />In France, there is a national health program for illegal immigrants called Aide médicale d'Etat (AME). The right wing government we had until May tried to limit the Aide Médicale d'Etat, introduced a fee (30 euros) and closed some centres helping the poor and the illegal immigrants. They pretended to speak in the name of the under middle class, since some people - especially the extremist populist party Front national - complained about paying for the public and for the private insurances whereas illegal immigrants could get free care. Fortunately, the actual left-wing government has a more human and rational approach. The fee was suspended. But for how long? <br /><br />The fee is by far not the only problem. Except in the medical centres dedicated to the poor, more and more physicians refuse to take care of illegal immigrants, saying they have problems to get their fee reimbursed by the public insurance system. The same happens for the poor (French and legal immigrants earning less than 650 euros a month) covered by another national program: Couverture maladie universelle (CMU). Most of the time, the real reason is that the public insurance does not reimburse the charges exceeding the statutory fees, asked by more and more physicians. So they prefer people having private insurances. Ethics doesn't matter much when it comes to money, in a market-driven system. <br /><br />An important point: estimations say most of the illegal immigrants (at least 70 to 80%) aren't informed about their rights, so they don't get any medical attention until they get to public hospitals with extremely severe conditions. Without prevention, without medical care, we see diseases like tuberculosis spread again. So even the ultracapitalists analysing only costs should recommend a health program for illegal immigrants, since costs are much higher for aggravating conditions treated in hospitals, for potential epidemics of infectious diseases and so on. Critics should think about this. (It's not my approach; I just try to give some arguments) <br /><br />A last point to complete the picture, now that I've said so much ;) <br /><br />The under middle class is in a difficult situation, too. The universal health insurance (régime général d'assurance-maladie) covers about 65 to 70% of the medical costs. Except for the poor covered by the CMU and for the illegal immigrants covered by the AME, everyone has to pay for a private insurance that covers the other 30%. <br />According to the last estimations, 29% of the French population (not including illegal immigrants) give up care because of financial costs, since they can't pay for a private insurance (and can't find physicians that don't charge excessive fees). <br /><br />I'm not optimistic at all about the future.
